As a Bilingual Nurse Case Manager, you will play a vital role in connecting patients with the resources they need to navigate their healthcare journey. You will serve as a dedicated advocate, providing support and guidance throughout the process.

Your Responsibilities:
  • Provide comprehensive support to patients and healthcare providers, including information on patient support services, specific diseases, and therapies based on manufacturer-provided materials and product monographs.
  • Collaborate with patients and families to conduct thorough needs assessments related to medication access.
  • Act as a liaison between patients, families, healthcare providers, and payers to facilitate medication access.
  • Answer general inquiries from patients via telephone or web-based platforms regarding program services, disease states, or product-related questions, drawing upon provided program materials.
  • Offer ongoing support and guidance on disease states or product-related questions based on program materials to ensure patient adherence as needed.
  • Refer patients to their healthcare providers for clinical questions, assessments, and situations that fall outside the scope of program materials or manufacturer product monographs.
  • Manage databases and conduct ongoing audits of files and captured information.
  • Adhere to established case management protocols, including data collection, ongoing patient follow-up, therapy disruption/discontinuation monitoring, adverse event reporting, and liaison with primary healthcare providers.
  • Work closely with reimbursement specialists and physicians to secure funding for patients' therapies; assist with alternative funding searches when necessary.
  • Prepare for and attend specialized training sessions for specific therapies provided by the pharmaceutical team as required.
  • Participate in pharmaceutical meetings as needed.
  • Attend patient meetings, conferences, and events to educate patients and provide information about Patient Support Program services.
  • Collaborate with patient associations and advocacy groups as required.
  • Build strong relationships with affiliated companies to ensure seamless coordination of care.
Your Qualifications:

We are seeking a compassionate and dedicated individual with a strong commitment to patient well-being. The ideal candidate will possess:

  • A nursing degree from an accredited institution.
  • Current licensure as a Registered Nurse (RN).
  • Experience in case management or a related healthcare field.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Bilingual proficiency in English and [Target Language].
