Position Overview
We are currently seeking a Diabetes Educator (Registered Dietitian) to join our team on a temporary, full-time basis at our Barrie location. This role is designed as a hybrid position, adapting to clinic requirements.
Key Responsibilities
- Provide education and guidance to patients on self-management techniques in both individual and group formats, primarily focusing on diabetes and, secondarily, on other health issues such as general health and metabolic conditions (including weight management and obesity).
- Collaborate with the multidisciplinary team to develop personalized care plans for patients.
- Document detailed patient interactions within the EMR system.
- Instruct patients on effective insulin administration, including titration and management, in accordance with medical directives.
- Teach patients self-monitoring of blood glucose (SMBG) as per medical directives.
- Conduct patient follow-ups through various channels (in-person, email, telephone, video).
- Gather comprehensive health histories from patients.
- Offer consultations to community groups, partners, and team members as needed.
- Provide training for new staff and educators as required.
- Communicate effectively with patients, families, caregivers, and healthcare professionals.
- Utilize information technology appropriately for patient care, including statistical tracking.
- Stay informed about new, evidence-based patient assessment and treatment methods.
- Assist with research initiatives, including providing dietary counseling for research participants.
- Conduct evening virtual workshops as necessary.
- Participate in a minimum of two community outreach events annually.
- Complete annual outreach calls.
- Aid in program implementation by reviewing patient charts, entering data, and adhering to program guidelines.
- Manage administrative tasks, which may be delegated to administrative staff, volunteers, or interns, including rescheduling appointments, creating workshop materials, ordering supplies, tracking patient visits, and entering program data.
- Engage in Journal Clubs and other collaborative learning opportunities.
- Contribute to the dissemination of the program's vision.
- Support and promote LMC initiatives that are not funded by the ministry, including both paid and unpaid services.
- Experience in motivational interviewing and evidence-based coaching techniques is an asset.
- Health coaching experience is valued, or willingness to undergo training as a health coach.
Qualifications
- A valid RD license in good standing with the College of Dietitians of Ontario.
- Experience in teaching and counseling individuals and groups.
- Must obtain or be pursuing CDE designation.
- A valid driver's license in good standing and access to a personal vehicle.
