We are seeking a Registered Practical Nurse (RPN) to take on the role of Community Care Supervisor within our organization.
The Community Care Supervisor is responsible for overseeing a dedicated team of Personal Support Workers (PSWs), ensuring they deliver high-quality personal care to clients in their residences.
Key Responsibilities:- Supervise and guide a team of Personal Support Workers to ensure exceptional personal care is provided to clients in their homes.
- Conduct visits to clients' residences and various care settings.
- Collaborate effectively with multidisciplinary teams across different care environments.
- Evaluate client needs in accordance with established timelines, develop care plans, and monitor progress towards desired outcomes.
- Manage staff performance, including conducting evaluations and implementing necessary disciplinary measures.
- Collect, document, and analyze feedback from clients and staff, maintaining awareness of key performance indicators such as caseload volume and staff turnover.
- Address and document all client concerns and complaints promptly.
- Represent Spectrum Health Care by participating in local healthcare committees and community events.
- Engage in ongoing professional development through internal and external educational opportunities.
- Current registration with the Colleges of Nurses of Ontario in good standing.
- A minimum of three (3) years of progressive leadership experience in a supervisory role, ideally within a community care setting.
- Strong interpersonal skills with the ability to manage challenging situations effectively.
- Proficient in report writing and effective verbal and written communication.
- Excellent command of the English language, both spoken and written (additional language skills are an asset).
- Comprehensive understanding of community nursing principles, service delivery, and program development.
- Possession of a valid driver's license and access to a reliable vehicle.
- Basic proficiency in Microsoft Word and Excel, with a willingness to learn proprietary software.
