About the Role:

Key Responsibilities:

  • Conduct thorough assessments of patients' needs and develop personalized care plans.
  • Provide skilled nursing care to patients in various settings, including their homes.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Maintain accurate and up-to-date patient records.

Requirements:

  • Current registration with the CNO as a Registered Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Reliability, flexibility, and a passion for making a difference in your community.
