About the Role

VHA Home HealthCare is seeking a skilled Registered Practical Nurse (RPN) to join our team as a MGH2Home Program Coordinator. In this role, you will provide and coordinate home health nursing services to diverse individuals, families, and groups, adhering to the College of Nurses of Ontario Practice Standards and VHA's policies and procedures.

Key Responsibilities
  • Assess, monitor, and make care planning/coordinating decisions
  • Evaluate care plan interventions through ongoing reassessment and evaluation results
  • Ensure the quality of care by identifying and acting on factors affecting practice autonomy and delivery of quality care
  • Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families
  • Support client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement
  • Inform nursing supervisors of client activities and relevant concerns or changes in the client or the client care plan
  • Maintain documentation standards according to the College of Nurses of Ontario
Requirements
  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Membership of your Professional Association preferred (WeRPN)
  • RPN diploma/degree from a recognized university or college
  • Valid First Aid or CPR Certification
  • Acute Care and Medical-Surgical nursing experience is an asset
  • Competency in IV Infusion is preferred
  • Knowledge of community nursing is an asset
  • Demonstrate effective communication (written and verbal) and documentation skills
  • Demonstrate reliable attendance, punctuality, and availability for the role
  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care
  • A valid Ontario driver's license (G License) and a reliable vehicle for traveling is an asset
