About the Opportunity

The Care Coordinator is responsible for the assessment, care planning, and authorization of services for clients referred to Continuing Care. This role involves providing an array of services and programs to assist individuals and families cope with complicated acute or chronic health situations in the community.

The Care Coordinator assesses clients' needs, identifies goals, and develops plans to meet those goals. They work with interprofessional teams to facilitate hospital discharge and return to the community. This role requires strong communication and negotiation skills, as well as the ability to work independently and collaboratively in a busy multidisciplinary environment.

The Care Coordinator is responsible for:

  • Assessing clients' needs and developing care plans
  • Authorizing services and making referrals to other professional and volunteer resources
  • Working with interprofessional teams to facilitate hospital discharge and return to the community
  • Maintaining ongoing communication with clients, family, caregivers, care providers, and healthcare teams

This role requires a strong understanding of community agencies and resources, as well as excellent assessment, interviewing, and negotiation skills. The Care Coordinator must be able to work independently and collaboratively in a busy multidisciplinary environment.

About You

We are seeking a highly skilled and experienced Care Coordinator to join our team. The ideal candidate will have:

  • A Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration (or eligibility for registration) with the applicable Nova Scotia Professional Regulatory body
  • Minimum three (3) years' experience with the professional regulatory body
  • Demonstrated knowledge and/or experience in a community or long-term care setting
  • Knowledge and/or experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and collaboratively in a busy multidisciplinary environment
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset

Hours
  • Permanent Full Time; 70 Hours Bi-weekly
  • Shifts may include days, evenings, weekends, and holidays
Compensation and Benefits

$46.75 Hourly
