Position Overview

Join our team as a Community Care Assistant with Vancouver Coastal Health (VCH)

Community Care Assistants play a vital role in our communities, providing essential support that greatly enhances the quality of life for clients and patients.

In your role as a Community Care Assistant at Vancouver Coastal Health, you will:

Operate under the guidance of the Supervisor/Coordinator/Manager or their delegate to deliver home support services to clients. Assist clients with varying levels of care, monitoring and reporting any significant changes in their condition. Collaborate with the healthcare team, clients, and their families to promote independence in daily living activities. Encourage and motivate clients to engage actively in their care. Provide valuable input during the care planning process to the healthcare team.

ABOUT HOME SUPPORT SERVICES:

Home Support Services offer a variety of assistance options aimed at helping individuals with ongoing health conditions maintain their independence and safety at home. Our Home Care Teams provide case management and nursing services, ensuring clients receive care with compassion and respect.

Qualifications

Education & Experience

Completion of a recognized Health Care Assistant Program. Active registration with the BC Community Health Worker and Care Aide Registry. Experience working with individuals with dementia, geriatric, or palliative care needs is advantageous. A valid BC Driver's license and access to a vehicle are required for local travel.

Knowledge & Skills

Understanding of client-centered care and promoting independence. Knowledge of safe body mechanics and injury prevention techniques. Familiarity with operating mechanical lifts while adhering to safety protocols. Awareness of workplace violence prevention and personal safety strategies. Understanding of patient-centered care, particularly for individuals with dementia. Ability to apply a harm reduction approach in practice. Basic technological proficiency, including the use of computerized client care systems. Effective communication skills, both verbal and written, through various mediums. Knowledge of self-care and motivational support strategies. Ability to provide culturally safe and competent care, especially when working with Indigenous populations. Capacity to prioritize tasks within a team environment. Ability to follow directions from the healthcare team and clients. Skills in home management tasks such as cleaning and meal preparation. Capability to inspire clients and support their progress towards therapeutic goals. Physical ability to fulfill the responsibilities of the position.

Additional Information

Work hours, including days off and work locations, may vary based on operational needs. Successful candidates may need to undergo a Criminal Records Review Check.

As per current Public Health Orders, all employees of Vancouver Coastal Health must be fully vaccinated against COVID-19 or have received the most recent updated vaccine. Proof of vaccination status will be required.
