Job Title: Transitional Worker

Job Summary:

We are seeking a highly motivated and compassionate Transitional Worker to join our team at Southeast Child Family Services. As a Transitional Worker, you will play a vital role in supporting young adults in their transition to independence, fostering a positive and healthy environment that promotes their holistic development.

Responsibilities:

  • Support young adults in developing and achieving their personalized goals and life plans, providing comprehensive and continuous wrap-around supports.
  • Assist young adults in accessing essential services, including food, clothing, transportation, medical and optical needs, therapy, and culturally relevant healing services.
  • Facilitate connections with land, culture, language, family, and community, and coordinate substance use treatment and recovery resources.
  • Ensure cleanliness and organization within transitional housing facilities, promoting a conducive living environment.
  • Organize outings and accompany young adults to appointments, assisting with case management tasks as needed.

Qualifications:

  • Post-secondary degree/diploma in social services or a related field.
  • Knowledge of Indigenous child welfare practices, The Child and Family Services Act, and An Act respecting First Nations, Inuit and Metis children, youth and families.
  • Demonstrated understanding and appreciation of Anishinaabe cultural beliefs, values, and norms.
  • Experience working with trauma, crisis, addictions, family violence, child abuse/neglect, grief, and loss issues.
  • Excellent verbal and written communication skills, with proficiency in Microsoft Office.
  • Valid driver's license and reliable vehicle.
  • Satisfactory Criminal Record, Prior Contact; and Child Abuse Registry Checks.
