At Southeast Child Family Services, we are seeking a dedicated and compassionate Transitional Worker to join our team.

Job Summary:

The Transitional Worker will play a vital role in fostering a positive and healthy environment for young adults in our care. This includes supporting their personal growth, independence, and well-being through various programs and services.

Key Responsibilities:

  • Develop and implement personalized support plans for young adults, ensuring their needs are met and goals are achieved.
  • Provide wrap-around services, including access to therapy, legal support, and pre-employment training.
  • Assist with daily living tasks, such as food, clothing, and transportation needs.
  • Facilitate connections with family, community, and cultural resources.
  • Coordinate outings and appointments, and provide case management support as needed.

Qualifications:

  • Post-secondary degree in social services or a related field.
  • Knowledge of Indigenous child welfare practices and Anishinaabe cultural beliefs.
  • Experience working with young adults, particularly those with trauma or addiction issues.
  • Excellent communication and interpersonal skills.
  • Valid driver's license and reliable vehicle.
