Transitional Support Specialist

About the Role:

The Transitional Worker will play a vital role in fostering a positive and healthy environment for young adults in our care. This position requires a strong understanding of social services and the ability to provide comprehensive support to participants.

Key Responsibilities:

  • Support young adults in developing and achieving their personal goals, including accessing education, employment, and life skills training.
  • Provide case management services, including connecting participants with community resources and services.
  • Ensure the cleanliness and organization of transitional housing facilities, promoting a safe and healthy living environment.
  • Organize outings and accompany participants to appointments, providing transportation and support as needed.
  • Collaborate with Agency units to ensure seamless program involvement and services.

Qualifications:

  • Post-secondary degree or diploma in social services or a related field.
  • Knowledge of Indigenous child welfare practices and Anishinaabe cultural beliefs and values.
  • Experience working with young adults, particularly those with trauma or addiction issues.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Valid driver's license and reliable vehicle.
  • Satisfactory Criminal Record, Prior Contact; and Child Abuse Registry Checks.
