Job Summary:

We are looking for a detail-oriented and compassionate LPN to provide high-quality medical care to our clients. The successful candidate will have experience in community health and be able to work independently in a dynamic environment.

Key Responsibilities:
  • Provide medical care to clients in their homes and other settings
  • Manage diabetes, dialysis, medication, palliative care, phlebotomy, and wound care
  • Work collaboratively with other healthcare professionals to ensure comprehensive care
  • Document client information and progress
  • Participate in ongoing education and training to stay up-to-date on best practices
Requirements:
  • Current licensure as a Licensed Practical Nurse in the province of practice
  • Experience in community health an asset
  • Valid driver's license and access to a reliable vehicle
  • Current and original copy of a satisfactory Criminal Records Check
  • Ability to wear Personal Protective Equipment (PPE)
