Nurse Practitioner

We are seeking a skilled and compassionate Nurse Practitioner to join our interdisciplinary team at Flemingdon Health Centre. As a key member of our primary care team, you will provide comprehensive health care services to clients across all sites, with a focus on community-based, client-centered care.
Responsibilities:- Provide comprehensive primary health care services, including assessments, diagnosis, treatment plans, follow-up, referral, education, and referral within the scope of practice of a registered nurse in extended class.
- Maintain accurate client visits and findings on current record system.
- Coordinate client care within FHC and HATP and with other health care agencies and specialists.
- Determine the need for and order and interpret screening and diagnostic laboratory tests, chest and limb x-rays, diagnostic ultrasounds, etc.
- Counsel and provide information and follow-up on abnormal results.
- Prescribe designated drugs as specified by the College of Nurses of Ontario.
- Participate in the development and delivery of health services provided through HATP, including HATP primary care clinic (cradle to grave care), primary care for youth (ages 12 – 25) at the Youth Wellness Hub, and wrap-around support with Thorncliffe Park community health service providers.
- Follow clients in the community on scheduled home visits for clients unable to visit HATP locations.
- Participate in Quality Improvement Programs.
- Registered Nurse with the College of Nurses of Ontario in the Extended Class.
- Baccalaureate in Nursing and graduate of recognized Educational Program for Primary Care Nurse Practitioners.
- 2-3 years of experience in primary care or community nursing will be an asset.
- An ability to work as a member of an interdisciplinary health care team, responsible and accountable for own practice.
- Demonstrated experience in chronic disease management and prevention, and a good understanding of the role of health promotion.
- Experience working with marginalized communities and complex clients.
- Commitment to anti-racism and anti-oppression practices.
- Excellent oral and written communication skills.
- Ability to work flexible hours, evening and weekend work expected in alignment with HATP's Strategic Priority of Catalyzing Community Driven Care and increasing access to health services and supports for equity-deserving community members.
- Ability to speak other languages, preferably Urdu, Slovak, or Arabic is an asset.
- Knowledge of EMR System an asset (Practice Solutions).
