Flemingdon Health Centre is seeking a skilled Nurse Practitioner to join our interdisciplinary team. As a key member of our primary care team, you will provide comprehensive health care services to clients across all sites. This is an exciting opportunity to work in a dynamic and community-driven environment, focused on improving health outcomes and reducing barriers to care.
Responsibilities:
- Provide comprehensive primary health care services, including assessments, diagnosis, treatment plans, and follow-up care.
- Coordinate client care within Flemingdon Health Centre and with other healthcare agencies and specialists.
- Develop and implement health promotion and education programs to support clients and the community.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Collaborate with other healthcare professionals to provide comprehensive care and support to clients.
Requirements:
- Registered Nurse with the College of Nurses of Ontario in the Extended Class.
- Baccalaureate in Nursing and graduate of a recognized educational program for primary care nurse practitioners.
- 2-3 years of experience in primary care or community nursing is an asset.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work flexible hours, including evenings and weekends, as required.
