Position Overview:

The role of the Care Team Coordinator is vital in ensuring the delivery of high-quality care at Closing the Gap Healthcare Group. This position involves overseeing client interactions and coordinating services with our healthcare providers.

Work Schedule:

  • Days: Monday to Friday
  • Shifts: 2 shifts per week, including every other weekend and a few weekdays
  • Available Shifts: 7am – 3pm, 8am – 4pm, 1pm – 9pm
  • Work Environment: On-Site

Key Responsibilities:

  • Coordinate visits and maintain communication with service providers.
  • Deliver empathetic client service while managing administrative tasks effectively.
  • Assess client requests promptly and accurately, escalating issues as necessary.
  • Schedule and coordinate service providers to align with client needs.
  • Provide comprehensive information about CTG programs and services to clients.
  • Perform data entry tasks using the CTG computer system.
  • Support both internal and external customers regarding CTG services.

Ideal Candidate Profile:

  • Demonstrates a strong commitment to client-centered service.
  • Possesses excellent verbal and written communication skills.
  • Exhibits meticulous attention to detail and efficiency in data management.
  • Has knowledge of medical terminology (preferred).
  • Is proficient in computer and phone usage.
  • Holds a diploma in Business Administration, Office Administration, or Medical Office Administration, or has relevant experience with a Grade 12 diploma.
