Clinical Social Worker

We are seeking a skilled Clinical Social Worker to join our team at Ridge Meadows Hospital in Maple Ridge, BC. As a Clinical Social Worker, you will work collaboratively with our interdisciplinary team to provide high-quality psychosocial assessments and interventions to patients and families. Your expertise in counseling and case management will enable you to promote effective coping and adaptation methods, and support clients in achieving optimal functioning and mastery of situations.
Responsibilities- Conduct comprehensive social and psychosocial assessments of patients and families to identify their needs and develop effective care plans.
- Provide individual, group, and family counseling services to promote emotional and social well-being, and support clients in achieving their goals.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients and families.
- Act as a resource for clients and families by providing information and referrals to community resources, and facilitating access to these resources.
- Participate in quality improvement activities to enhance the delivery of psychosocial services and promote best practices in clinical social work.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two (2) years' recent related clinical social work experience working with individuals, groups, and families.
- Current full registration with the BC College of Social Workers.
