We are seeking a dedicated and compassionate Social Worker to join our team at Fraser Health Authority. As a key member of our interdisciplinary team, you will provide clinical social work counseling services to clients and families with serious, persistent mental illness and substance use in the Assertive Community Treatment (ACT) program.
Key Responsibilities- Conduct comprehensive social work and psychosocial assessments of clients with mental illness and substance misuse/dependence.
- Provide therapeutic clinical counseling services to clients with mental illness and their families.
- Educate and provide consultation and support to clients, families, and healthcare professionals on issues related to high risk factors, recovery, and current community practice in the field of mental health.
- Maintain client records by documenting client assessments, charting client information, and preparing progress notes and reports.
- Collaborate with hospital and community services from a social work perspective to determine appropriate levels of care and the effective utilization of client resources.
- Masters Degree in Social Work (MSW) from an accredited school of Social Work.
- Two (2) years' recent, related experience in a mental health treatment setting or an equivalent combination of education, training, and experience.
- Current full registration with the BC College of Social Workers.
