Clinical Social Worker

We are seeking a highly skilled Clinical Social Worker to join our team at Fraser Health Authority. As a Clinical Social Worker, you will play a vital role in providing social work services to improve the emotional and social consequences of a health condition for our clients and their families.
Key Responsibilities:- Conduct social and psychosocial assessments of patients and families to identify their needs and develop a comprehensive care plan.
- Provide therapeutic counseling services to clients and families experiencing a variety of circumstances related to their illness.
- Facilitate a process whereby the patient and family identify their needs for recovery, adaptation, and mastery of the situation.
- Collaborate with other healthcare professionals to develop a comprehensive plan of care and provide ongoing monitoring of psychosocial situations for clients.
- Act as a resource for clients and their families by providing information regarding community resources and access to these resources.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two (2) years' recent related clinical social work experience working with individuals, groups, and families.
- Current full registration with the BC College of Social Workers.
