About the Role

VHA Home HealthCare is seeking a skilled Care Team Supervisor to join our team. As a key member of our leadership team, you will be responsible for overseeing the operations and people management of our geographical teams.

Key Responsibilities
  • Lead and manage a team of scheduling coordinators, clients, and field staff to achieve desired performance outcomes.
  • Ensure clients receive evidence-informed quality care and population-specific quality outcomes, with a focus on client and staff safety.
  • Supervise and support the team in adhering to VHA Home HealthCare policies and procedures, key performance indicators, contractual requirements, and regulatory standards.
  • Monitor resource utilization, resolve rejected billings, and ensure fiscal accountability.
  • Collaborate with other teams and regions to achieve organizational performance targets.
Requirements
  • RN or RPN with CNO registration in good standing.
  • Minimum of 3 years of experience in community health, preferably with supervisor experience in healthcare.
  • Strong leadership and mentoring skills, with the ability to work collaboratively within an interprofessional care team.
  • Effective written communication skills and clinical documentation.
  • Computer literacy and Microsoft Office applications skills.
  • Proven ability to set, monitor, and achieve personal and organizational performance targets.
