Job Title: Care Team Supervisor

VHA Home HealthCare is a leading provider of home care services in Ontario, with a team of over 3,000 dedicated professionals. As a Care Team Supervisor, you will play a critical role in ensuring the delivery of high-quality, client-centered care to those who need it most.

Key Responsibilities:
  • Lead a team of scheduling coordinators and field staff to ensure seamless care delivery
  • Develop and implement operational plans to achieve quality and performance targets
  • Collaborate with regional teams to ensure effective communication and coordination
  • Monitor and analyze data to inform decision-making and drive continuous improvement
  • Supervise and mentor staff to achieve desired performance outcomes
Requirements:
  • RN or RPN with CNO registration in good standing
  • Minimum 3 years experience in community health, with 2-3 years of supervisor experience in healthcare
  • Strong leadership and communication skills, with ability to work collaboratively in an interprofessional team
  • Effective written and verbal communication skills, with ability to document clinical information
  • Computer literacy and proficiency in Microsoft Office applications
