Sunrise Senior Living is seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for providing exceptional care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.
Key Responsibilities- Build Meaningful Relationships: Develop strong relationships with residents and their families, providing assistance with activities of daily living, attending to individual care needs, and getting to know their unique preferences and personalities.
- Notify Management of Changes: Inform management of any changes in a resident's condition and recommend adjustments in the level of care and service as needed.
- Assist with Life Skills: Assist residents with life skills and other life-enriching activities as indicated on their individual profile.
- Blends Multisensory Experiences: Create a variety of multisensory experiences for residents to enjoy.
- Develop Individualized Service Plans: Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
- Responsible for Resident Care: Be responsible for a designated group of residents during the shift, knowing their whereabouts and physically checking on them throughout the shift.
- Partners with Community Team: Collaborate with the community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies.
- Respond to Dining Needs: Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
- Invite, Encourage, Assist: Invite, encourage, and assist residents in life skills and other life-enriching activities as indicated on their ISP and demographic profile.
- Dedication to Serving Seniors: Demonstrate a passion for serving seniors with excellent customer service skills.
- Positive Attitude and Flexibility: Possess a positive attitude, flexibility to perform various duties, and the ability to work in a team environment.
- Education and Certifications: High School diploma/GED accepted and may be required per state regulations. In states where appropriate, must maintain certifications.
- Age Requirement: Must be at least 18 years of age.
- Previous Experience: Previous experience working with seniors preferred.
- Decision-Making Skills: Ability to make choices, decisions, and act in the resident's best interest.
- Communication and Organizational Skills: Possess written and verbal skills for effective communication and a level of understanding, as well as competent organizational and time management skills.
