About the Role

We are seeking a highly skilled and compassionate Case Manager to join our team at the Canadian Mental Health Association. As a Case Manager, you will play a critical role in supporting individuals with mental health issues, promoting their recovery, and empowering them to achieve their full potential.

Key Responsibilities
  • Work collaboratively with clients to assess, plan, and provide treatment, rehabilitation, and support services.
  • Develop and implement individualized service plans to maximize clients' independence and potential.
  • Provide education and support to clients and their families on mental health issues, treatment options, and community resources.
  • Collaborate with healthcare professionals, community agencies, and other stakeholders to ensure comprehensive care and support.
  • Participate in individual and systemic advocacy initiatives to promote clients' rights and interests.
Requirements
  • Master's degree in a related field (e.g., social work, psychology, nursing) or equivalent experience.
  • Current registration with a regulatory body (e.g., OCSWSSW, College of Nurses).
  • Experience working with individuals with mental health issues, particularly those with early psychosis.
  • Knowledge of mental health reform principles, the Ontario Mental Health Act, and relevant legislation.
  • Ability to apply recovery principles and empowerment-oriented philosophies and practices in work with consumers.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
