Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at the Canadian Mental Health Association. As a Case Manager, you will play a critical role in supporting individuals with mental health challenges and involvement in the justice system.
Job Summary:The Case Manager will work collaboratively with individuals, their families, and community agencies to develop and implement personalized plans to address their mental health needs and reduce the likelihood of further conflict with the law.
Key Responsibilities:- Conduct comprehensive assessments and develop individualized plans to address mental health needs and reduce recidivism
- Collaborate with community agencies, healthcare providers, and justice system stakeholders to ensure seamless service delivery
- Provide education and support to individuals and their families to promote mental health recovery and well-being
- Develop and maintain relationships with community resources and services to ensure access to necessary support
- Monitor and evaluate the effectiveness of individualized plans and make adjustments as needed
- Master's degree in a related field (e.g., social work, psychology, nursing)
- Minimum 2 years of experience working in a community-based mental health setting
- Knowledge of the Ontario Mental Health Act and mental health reform principles
- Ability to work collaboratively with diverse stakeholders and populations
- Strong communication and interpersonal skills
