Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team in the Mental Health and Justice Program. As a Case Manager, you will work closely with adults and youth who have mental health challenges and are involved in the criminal justice system.
Key Responsibilities:- Collaborate with individuals to identify strengths and needs, and develop long-term goals and short-term objectives.
- Assist individuals in acquiring skills and connecting with formal and informal support, peer support, professional services, and resources.
- Navigate the court system and provide support through information, referral, and consultation.
- Work directly with court staff to explore possible outcomes through the court process.
- Degree or diploma in health or social services.
- Experience in a community-based mental-health setting, with experience with the criminal justice system an asset.
- Current registration with the OCSWSSW or a regulatory body under the Regulated Health Professions Act.
- Knowledge of systemic issues and social determinants of health.
- Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with consumers.
Office work, community settings, meetings in clients' homes, and travel with clients.
