We are seeking a highly skilled and enthusiastic Travel Nurse Practitioner to join our team at RhynoCare in a Long-term Care (LTC) setting. As a key member of our interprofessional team, you will provide primary health care to residents, manage acute and chronic medical conditions, and promote health and wellness.
Responsibilities:- Deliver comprehensive health services to residents, including health assessments, diagnosis, and treatment.
- Provide on-call coverage for duties and schedules as required.
- Lead clinical assessments and make sound clinical judgments to ensure high-quality care.
- Collaborate with the LTC team to develop person-centered care plans and promote continuity of care.
- Communicate effectively with residents, families, and healthcare teams to ensure seamless care transitions.
- Participate in quality improvement initiatives and contribute to the development of systems to promote resident care.
- Demonstrate active participation in the CNO Quality Assurance Program to maintain continuing competence.
- Work collaboratively with the leadership team to develop educational programs for the care team.
- College of Nurses of Ontario registration in the Extended Class.
- Bachelor's degree in Nursing (BScN); Master's degree in Nursing (MSN) with a Nurse Practitioner specialization preferred.
- Membership with Nurse Practitioners' Association of Ontario (NPAO) (preferred).
- Has acquired professional liability insurance (with either RNAO or other liability insurance).
- National certification in gerontology is an asset.
- Ability to manage crises effectively and professionally.
- Excellent written skills and report writing for a clinical setting.
- Ability to communicate with clients in a non-judgmental manner.
- Basic CPR certification required.
- Must show evidence of valid current Vulnerable Sector Check (VSC).
- Other immunizations as mandated by the Ministry of Long-term Care (MoLTC).
