Job Title: Registered Practical Nurse - Reactivation Care Centre

North York General Hospital is seeking a highly skilled Registered Practical Nurse to join our Reactivation Care Centre team. As a key member of our interdisciplinary team, you will provide exceptional patient care and support to our patients and families.

Job Summary:

The Registered Practical Nurse will be responsible for providing nursing care to patients in the Reactivation Care Centre, with a focus on assessment, education, prevention, maintenance, and restoration of patient health. You will work collaboratively with an interdisciplinary team to ensure that patients receive comprehensive care and support.

Key Responsibilities:
  • Provide nursing care to patients in the Reactivation Care Centre, including assessment, education, and support.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct health assessments and provide education to patients and families on health promotion and disease prevention.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
Requirements:
  • Registered Practical Nursing diploma from a recognized Community College.
  • Registered and in good standing with the College of Nurses of Ontario.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • Minimum 2 years of recent Medical/Surgical and/or clinical nursing experience working with older adult, dementia, and cognitively impaired population preferred.
  • Recent health assessment course.
  • Bilingualism in French/English an asset.
