In alignment with the organization's established vision, values, and strategic objectives, the Community Health Worker II, as a member of a collaborative healthcare team, operates under the guidance of the Team Lead or designated clinical supervisor to deliver home support services to clients based on a predetermined care plan.
Responsibilities include assisting with daily living activities such as personal hygiene, meal planning and preparation, while fostering maximum independence.
You will also perform delegated tasks for which appropriate training has been completed.A vital aspect of this role is to ensure the well-being of individuals and families, promoting independent living within the community.
Updated Wage :
The new wage is: $29.60/hour
Work Schedule: Days, 10:00 to 14:54, Rotating
About Northern Haida Gwaii Hospital
This facility is a four-bed acute care hospital with additional long-term care and assisted living units. They provide a range of services including emergency care, laboratory services, diagnostic imaging, pharmacy, and community care services, along with mental health and substance use support, chronic disease management, physiotherapy, and speech therapy. Visiting specialists include Pediatrics, Orthopaedics, ENT, Nephrology, Endocrinology, Respirology, Rheumatology, Urology, and Ophthalmology.

- Valid Class 5 BC Driver's License.
