About the Role

We are seeking a skilled Registered Practical Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to patients in their homes, working collaboratively with our team to deliver comprehensive care.

Key Responsibilities
  • Conduct scheduled visits in the community to assess patient needs and provide care within your scope of practice.
  • Develop and implement individualized care plans, working closely with patients and their families to ensure their needs are met.
  • Provide education and support to patients and their caregivers, promoting independence and self-management.
  • Collaborate with our interdisciplinary team to coordinate care and ensure seamless transitions.
  • Document patient progress and outcomes, maintaining accurate and up-to-date records.
Requirements
  • Current registration with the College of Nurses of Ontario as a Registered Practical Nurse in good standing.
  • Valid driver's license and access to a reliable vehicle.
  • Experience in community care or a related field is an asset.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
