Nurse Case Manager

Vancouver Coastal Health is seeking a skilled Nurse Case Manager to join our team in the Home Health department at Pacific Spirit Community.
Key Responsibilities- Manage a caseload of clients with chronic illnesses or long-term physical disabilities, providing comprehensive care and support.
- Conduct assessments, identify problems, and develop plans of care in collaboration with interdisciplinary teams and community agencies.
- Provide continuity of care across the continuum, coordinating with healthcare professionals and community resources to ensure seamless transitions.
- Develop and maintain partnerships with General Practitioners and other healthcare providers to ensure comprehensive care.
- Act as a clinical resource for the interdisciplinary team, providing education and support on chronic disease management and self-care principles.
- Consult and collaborate with multidisciplinary teams, healthcare professionals, and community agencies to achieve excellence in client-centered care.
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Three years' recent clinical nursing experience in acute care, case management, or home care, or an equivalent combination of education and experience.
- Valid BC Driver's License and ability to travel locally.
- Demonstrated ability to problem-solve and think critically in a fast-paced environment.
- Comprehensive knowledge of chronic disease management and self-care principles.
- Ability to work independently and prioritize care for a caseload of clients.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
