Nurse Clinician Case Manager for the Neuromotor Program at BC Children's Hospital and SunnyHill Health Centre in Vancouver, BC. This role involves coordinating patient care, developing individualized service plans, and providing education to patients and families.
**Key Responsibilities:** * Coordinate patient assessments, respond to patient and family inquiries, and provide anticipatory guidance and case management. * Direct the intake/access process for the NMP team, including receiving and documenting child-specific information, conducting interviews, and assessing referrals. * Collaborate with healthcare teams to evaluate patient management and develop individualized service plans. * Coordinate teaching of patients and families with new Neuromotor condition and/or sensory impairment diagnosis. * Provide supportive case management to families, including patient advocacy, information, and referrals to community services. * Liaise with community health and educational services and agencies to clarify information and responsibilities. * Establish a collaborative working relationship with Sunny Hill and BCCH staff and community-based healthcare and service providers. * Develop policies, procedures, and operational requirements related to the function of the NMP teams. * Participate in quality management activities related to the function of the Neuromotor Program and nursing practice. * Collaborate with interdisciplinary teams and nursing colleagues on research-oriented opportunities. * Act as a clinical nursing resource for patients, families, nursing staff, healthcare professionals, and community agencies. * Use the nursing process and specialized nursing knowledge to provide direct patient care and case management. **Qualifications:** * Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nursing Professionals (BCCNP). * Three years recent related and progressively responsible experience, including previous experience working with the population and/or in a child development and rehabilitation setting. * CPR Training - Basic Cardiac Life Support (BCLS) - Level I. * Valid BC Driver's License and access to vehicle and ability to travel. **Skills and Knowledge:** * Knowledge of community-based services for children with Neuromotor/developmental conditions. * Excellent communication skills, both oral and written. * Strong leadership and organizational skills. * Ability to relate effectively to children and their families, dealing effectively with those experiencing stress and/or in crisis. * Ability to collaborate and work effectively as a member of an interdisciplinary care team and function as a nursing resource. * Ability to plan, deliver, and evaluate educational sessions based on principles of adult education. * Ability to identify and resolve problems using therapeutic communication strategies. * Ability to work independently to organize and prioritize multiple tasks. * Physical ability to perform the duties of the job. * Strong computer skills, including Excel, Word, and PowerPoint, and Internet Browsers. * Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC and BC's Declaration on the Rights of Indigenous Peoples Act. * Knowledge of social, economic, political, and historical realities of settler colonialism on Indigenous Peoples and familiarity with addressing Indigenous-specific anti-racism, anti-racism, and Indigenous Cultural Safety and foundational documents and legislative commitments. **What We Offer:** * Join one of BC's largest employers with province-wide programs, services, and operations, offering vast opportunities for growth, development, and recognition programs that honour the commitment and contribution of all employees. * Access to professional development opportunities through our in-house training programs, including +2,000 courses, such as our San'yas Indigenous Cultural Safety Training course, or Core Linx for Leadership roles. * Enjoy a comprehensive benefits package, including municipal pension plan, and psychological health & safety programs and holistic wellness resources. * Annual statutory holidays (13) with generous vacation entitlement and accruement. * PHSA is a remote work-friendly employer, welcoming flexible work options to support our people (eligibility may vary, depending on position). * Access to WorkPerks, a premium discount program offering a wide range of local and national discounts on electronics, entertainment, dining, travel, wellness, apparel, and more.
