Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Fraser Health as a Fracture Liaison Service Nurse. This is an exciting opportunity to work in a dynamic and collaborative environment, providing person-centered care to clients with fragility fractures.

Key Responsibilities
  • Assess clients referred to the Fracture Liaison Service and develop comprehensive care plans to prevent future fractures.
  • Collaborate with the interprofessional care team to ensure seamless care coordination and delivery.
  • Provide education and support to clients and their families on fragility fracture prevention, health promotion, and secondary fracture prevention.
  • Screen eligible clients with low trauma fractures and assess referrals to the Fracture Liaison Service in accordance with established criteria.
  • Document assessments, nursing diagnoses, observations, interventions, client/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
Requirements
  • Graduation from an approved school of nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years of recent, related acute care experience working with a geriatric population experiencing or at risk of experiencing fragility fractures.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
Preferred Qualifications
  • Experience working in a geriatric population and knowledge of fragility fracture prevention and health promotion.
  • Ability to communicate effectively with clients, families/care givers, and members of the interprofessional care team.
  • Ability to promote person-centered care and demonstrate empathy for clients and families/care givers.
