Geriatric Nurse Clinician

We are seeking a highly skilled and experienced Geriatric Nurse Clinician to join our team at Fraser Health Authority. As a Geriatric Nurse Clinician, you will play a critical role in providing clinical expertise and consultation to our healthcare teams, focusing on the care of older adults with complex needs.
Key Responsibilities:- Provide clinical leadership and expertise in gerontological nursing, ensuring high-quality care for older adults.
- Develop and implement evidence-informed clinical decision support tools and policies.
- Collaborate with interprofessional teams to develop and support individualized care plans for older adults.
- Conduct comprehensive gerontological nursing assessments and ongoing evaluations of older adults with complex care needs.
- Role model direct care skills, coach, mentor, and teach nursing skills to staff and students.
- Develop and revise education tools and plans based on identified learner needs and chart audits.
- Deliver learner-centered education to individuals or groups of staff and patient/families.
- Participate in quality improvement and risk management activities, evaluating the effect of changes in practice and making recommendations for improvement.
- Assist in the review, development, implementation, and evaluation of evidence-informed clinical decision support tools and policies.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years of recent related clinical experience in acute gerontological nursing, including one (1) year of education and/or leadership experience.
- Completion of post-graduate coursework and/or certification applicable to gerontological nursing (Canadian Nurses Association Gerontology Nursing Certificate).
- Valid BC Driver's Licence and access to a personal vehicle for business-related purposes.
