About the Role:
The Clinical Lead for Assisted Living is responsible for overseeing the delivery of high-quality care services to residents in our Assisted Living communities. This role requires a strong leader who can motivate and support a team of caregivers to provide exceptional care and services.
Key Responsibilities:
- Leadership and Team Management: Lead and motivate a team of caregivers to achieve organizational goals and objectives.
- Care Planning and Coordination: Assess and maintain residents' Personal Service Plans, ensuring that their care needs are met and their rights are respected.
- Collaboration and Communication: Collaborate with Health Authority Case Managers, families, and other stakeholders to ensure seamless care coordination and communication.
- Caregiver Support and Development: Recruit, train, and support staff in daily care tasks, ensuring that they have the skills and knowledge needed to provide high-quality care.
- Quality Assurance and Improvement: Implement and review care plans for residents, ensuring that they are meeting the organization's quality standards and regulatory requirements.
- Environment of Care: Maintain a safe and respectful environment for all residents, staff, and visitors.
Qualifications and Skills:
- Education and Certification: Graduate of an approved Practical Nurses program and valid First Aid Certificate.
- Experience and Knowledge: Recent experience in LPN practice and knowledge of Assisted Living Regulations and Community Care Act.
- Personal Qualities: Good physical health, effective communication and interpersonal skills, willingness to work flexible hours, ability to handle challenging situations, and readiness to respond to emergencies.
