Nurse Practitioner

The Registered Nurse is a key member of the VON Canada team, responsible for providing high-quality, client-centered care to individuals and families in their homes and community. This role requires a strong commitment to delivering evidence-based care, working collaboratively with other healthcare professionals, and promoting health and well-being through education and support.
Key Responsibilities- Manage and coordinate care, evaluating health outcomes and educating clients and families to meet their health goals.
- Provide all levels of nursing care, from stable clients with predictable outcomes to those with acute, complex, and rapidly changing health needs.
- Develop and implement cost-effective, organized, and outcome-directed care plans, assigning and delegating tasks as necessary.
- Provide service consistent with VON and provincial regulatory body standards, incorporating current research findings into clinical practice.
- Assess client, family, and caregiver needs, developing care plans that promote independence and self-care.
- Implement nursing strategies and interventions according to established care plans, documenting client responses and revising plans as needed.
- Collaborate with management staff and colleagues to evaluate practice and maintain competence through continuing education and self-reflection.
- Licensed as a Registered Nurse in the province of practice.
- Canadian Nurses Association certification in Community Health Nursing an asset.
- Clinical practice experience and experience in community health an asset.
- Strong written and verbal communication skills, with ability to engage with clients, staff, and the public.
- Ability to work independently, problem-solve, and adapt nursing interventions as needed.
- Capacity to adhere to strict confidentiality and maintain professional boundaries.
