VON Canada is seeking a skilled Nurse Practitioner to join our team in Ontario. As a key member of our community care team, you will be responsible for providing high-quality, client-centered care to individuals and families in their homes and community.
Key Responsibilities:
- Manage and coordinate care, evaluating health outcomes and educating clients on health goals.
- Provide all levels of nursing care, from stable clients with predictable outcomes to those with acute, complex, and rapidly changing health needs.
- Develop and implement care plans, incorporating discharge planning as needed.
- Perform technical nursing skills safely and effectively, adhering to nursing principles and VON policies.
- Document nursing processes clearly and accurately, following regulatory and VON standards.
- Engage with clients, families, and caregivers, promoting independence and self-care.
- Collaborate with healthcare professionals and community organizations to promote best practices and improve client outcomes.
- Participate in quality improvement initiatives, seeking input from clients to ensure services meet their needs.
- Function in accordance with VON policies, procedures, and strategic direction.
- Other duties as assigned.
Requirements:
- Licensed as a Registered Nurse in the province of practice.
- Canadian Nurses Association certification in Community Health Nursing an asset.
- Clinical practice experience and experience in community health an asset.
- Strong written and verbal communication skills, with ability to engage with clients, staff, and the public.
- Ability to work independently, problem-solve, and adapt nursing interventions as needed.
- Capacity to adhere to strict confidentiality and maintain professional boundaries.
- Effective time management skills, with ability to plan and organize daily work activities.
