Job Title: Registered Practical Nurse (RPN) / Registered Nurse (RN)Job Summary:

We are seeking an experienced Registered Practical Nurse (RPN) or Registered Nurse (RN) to join our team as a dynamic and passionate member of our home and community care team. As a key member of our interdisciplinary care team, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Key Responsibilities:
  • Provide comprehensive care to clients in their homes and communities
  • Lead the care planning process and work with other disciplines to complete and implement interdisciplinary client care plans
  • Facilitate regular interdisciplinary team communication and lead discharge and transition care planning discussions
  • Support clients, families, and caregivers to develop strong self-care strategies and optimize their health through effective health promotion strategies
  • Collaborate with community support services to ensure seamless care transitions
Requirements:
  • Minimum of two years of nursing experience, preferably in home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
  • Canadian Nurses Association (CNA) community nursing certification is an asset
  • Ability to be flexible and adaptable with excellent organizational skills, case management, and working with/coordinating interdisciplinary care teams
  • Valid driver's license and a vehicle
