Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Nurse. As a Community Nurse, you will work with patients in their homes, clinics, and other community settings, providing exceptional nursing services and developing meaningful relationships with patients and their families.

Key Responsibilities
  • Develop and maintain effective care plans for patients in community settings
  • Provide nursing services in a variety of settings, including in-home, clinic, and retirement home settings
  • Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care coordination
  • Perform other clinical duties as required
