Job Title: Registered Practical Nurse BSO

St. George Community is seeking a highly skilled Registered Practical Nurse to join our Behavioural Support Team (BSO). As a key member of our team, you will play a critical role in providing exceptional care to our residents and their families.

About the Role:

The Registered Practical Nurse, BSO will be responsible for providing clinical leadership and expertise in the development and implementation of behaviour care plans. You will work closely with our Behavioural Support Team to assess and address the needs of our residents, providing comprehensive bio-psychosocial geriatric assessments, treatment, and follow-up planning.

Key Responsibilities:
  • Provide clinical leadership and expertise in the development and implementation of behaviour care plans.
  • Collaborate with the Behavioural Support Team to assess and address the needs of residents.
  • Conduct comprehensive bio-psychosocial geriatric assessments, treatment, and follow-up planning.
  • Develop and maintain a short-term active clinical caseload of older adults presenting with or at risk for responsive behaviours.
  • Provide coaching and training to LTC home staff on the use of frameworks such as PIECES and UFIRST.
  • Collaborate with external resources, including hospitals, CCAC, and primary care, to provide wraparound care.
  • Document all findings and interventions in a clear and concise manner in the client's clinical record.
  • Contribute to the development and implementation of quality improvement initiatives.
Requirements:
  • Registered Practical Nurse with current registration with the College of Nurses of Ontario.
  • Current CPR/First Aid certification.
  • Training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Dementiability.
  • Minimum one (1) year experience with clients in long-term care or residents in a long-term care home setting.
  • Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours.
  • Demonstrated ability to train, coach, and mentor others.
Preferred Qualifications:
  • Basic computer skills – MDS/RAI, Microsoft Word, Excel, and online learning utilities.
  • Experience with quality improvement methods and processes.
