We are seeking a highly skilled Registered Nurse/Registered Psychiatric Nurse to join our Community Mental Health team. As a key member of our interprofessional care team, you will provide treatment and care management services to individuals with mental illness and concurrent disorders.
Key Responsibilities- Coordinate care for clients by collaborating with the interprofessional care team, performing intake functions, conducting mental health assessments, and developing comprehensive treatment plans.
- Provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, and implementing care plans.
- Work from a strengths-based model, using motivational interviewing and engagement techniques to develop and implement individualized care plans.
- Establish and maintain therapeutic relationships with clients and their informal support network, promoting client self-determination and independence.
- Provide treatment services, including crisis intervention, individual counseling, and education, according to a variety of theoretical models.
- Initiate and participate in case conferences/meetings with clients, family, service providers, and members of the interprofessional team to ensure a coordinated approach to treatment.
- Consult and collaborate with members of the interprofessional care team, including MH&SUS Physicians, Primary Care Providers, and other health care professionals/providers.
- Monitor Extended Leave as required under the Mental Health Act, reviewing conditions of Extended Leave and determining if clients are following the conditions.
- Aid client access to services, including medical appointments, hospitals, and social programs, by providing healthcare information, contacting relevant programs/services, and assisting clients with processing forms and daily living tasks.
- Document assessments, care plans, observations, interventions, client/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Maintain a safe environment for clients, visitors, and employees by adhering to established safety and emergency practices and procedures.
Education, Training & Experience:
- Graduation from an approved School of Nursing or Psychiatric Nursing.
- Two (2) years recent related experience providing treatment services to individuals with mental health and/or concurrent disorders.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Valid Class 5 BC Driver's License and access to personal vehicle with appropriate insurance coverage.
