Job Summary:
The Sioux Lookout Meno Ya WIn Health Centre is seeking a skilled professional to fill the role of Acute Discharge Planner. As a key member of the Discharge Team, this individual will be responsible for assessing the psychosocial needs of patients and their families to develop, communicate, and implement effective and comprehensive plans for psychosocial interventions and complex discharges.
Key Responsibilities:
- Collaborate with the Discharge Team Lead to facilitate patient discharge within the organization.
- Coordinate patient repatriation from other facilities.
- Work with the Rehabilitation Team to provide individualized and tailored services to address client and family needs and preferences.
- Deliver case management services to patients in a hospital setting.
- Develop supportive programming to meet the needs of patients and families serviced.
- Facilitate professional development for providers in the areas of social support, quality of life, and end-of-life care.
- Perform post-discharge follow-up communication to evaluate services provided.
Requirements:
- Regulated healthcare professional as an RN/RPN in good standing with the College of Nurses of Ontario or Social Worker in good standing with the OCSWSSW or able to be registered.
- Demonstrated knowledge and practical experience in working with community organizations and resources.
- Demonstrated knowledge and practical experience in working with an interdisciplinary team.
- Demonstrated knowledge and practical experience with patient flow, including the inpatient unit and emergency department.
- Excellent interpersonal and leadership skills.
- Excellent communication skills, both written and verbal.
- Ability to work well independently.
- Knowledge and sensitivity to First Nations language and culture an asset.
- Experience working in a case management model of care an asset.
- Ability to speak Ojibway/Oji-Cree/Cree an asset.
