Job Summary:
The Sioux Lookout Meno Ya WIn Health Centre is seeking a skilled Acute Discharge Planner to join our team. As a key member of our Discharge Team, you will play a vital role in assessing the psychosocial needs of patients and families, developing and implementing effective discharge plans, and coordinating care transitions.

Key Responsibilities:
• Collaborate with interdisciplinary teams to assess patient needs and develop comprehensive discharge plans
• Coordinate patient repatriation from other facilities and facilitate care planning and discharges within the hospital setting
• Work with families and community agencies to navigate and improve transition between services for all patients
• Develop supportive programming to meet the needs of patients and families serviced
• Facilitate professional development for providers in the areas of social support, quality of life, and end-of-life care
• Perform post-discharge follow-up communication to evaluate services provided

Requirements:
• Regulated healthcare professional as an RN/RPN in good standing with the College of Nurses of Ontario or Social Worker in good standing with the OCSWSSW or other allied health professional with relevant skill set
• Demonstrated knowledge and practical experience in working with community organizations and resources
• Excellent interpersonal and leadership skills, with ability to work well independently
• Excellent communication skills, both written and verbal
• Ability to work collaboratively with interdisciplinary teams and adapt to changing priorities
• Knowledge and sensitivity to First Nations language and culture an asset
• Experience working in a case management model of care an asset
