Job Summary:
The Sioux Lookout Meno Ya WIn Health Centre is seeking a skilled and compassionate Acute Discharge Planner to join our team. As a key member of our Discharge Team, you will play a vital role in assessing the psychosocial needs of patients and families, developing and implementing effective discharge plans, and coordinating care transitions to ensure seamless transitions between hospital and community settings.

Key Responsibilities:
• Collaborate with interdisciplinary teams to assess patient needs and develop comprehensive discharge plans
• Coordinate patient repatriation from other facilities and facilitate care planning and discharges within the hospital setting
• Work cooperatively with the Rehabilitation Team to provide individualized services that address client and family needs and preferences
• Deliver case management services to patients in a hospital setting and to residents of the William A. George Extended Care Unit
• Develop supportive programming to meet the needs of patients and families serviced, including palliative care and cognitive supports
• Facilitate professional development for providers in the areas of social support, quality of life, and end-of-life care
• Perform post-discharge follow-up communication to evaluate services provided
• Adhere to organizational policies and procedures concerning infection control and attend training as necessary

Requirements:
• Regulated healthcare professional as an RN/RPN in good standing with the College of Nurses of Ontario or Social Worker in good standing with the OCSWSSW or other allied health professional with relevant skill set
• Demonstrated knowledge and practical experience in working with community organizations and resources
• Excellent interpersonal and leadership skills, with ability to work well independently
• Excellent communication skills, both written and verbal
• Ability to work collaboratively with interdisciplinary teams and coordinate care transitions
• Knowledge and sensitivity to First Nations language and culture an asset
• Experience working in a case management model of care an asset
• Ability to speak Ojibway/Oji-Cree/Cree an asset
