Halton Healthcare is seeking a skilled Social Worker to join our interdisciplinary Step-Up Neuro Rehab team. As a key member of our team, you will play a vital role in assessing, planning, implementing, and evaluating patient needs, as well as working with their family and/or significant others.
Key Responsibilities:- Conduct comprehensive social work assessments using a variety of modalities and in accordance with professional standards and clinical practices and policies.
- Consider the individual needs and readiness of patient/family/caregiver when providing resources and connecting them to community services.
- Provide education to stroke/neuro patients and their families regarding what to expect as they transition back into the community.
- Collaborate with the inter-disciplinary team to address ongoing patient needs, to participate in discussions to help optimize patient care and to ensure the coordination of community based support services.
- Engage patient to assist with adjustments related to socio/emotional health issues and identify patient's network of supports.
- Formulate a care plan to accomplish patient's goals.
- Resolve complex issues with patient/family through use of conflict resolution, providing mediation and advocacy both with internal and external partners.
- BSW or MSW required.
- Current registration and membership in good standing with the College of Social Workers and Social Service Workers.
- Minimum 3 years experience in a hospital or community health organization.
- Experience working in neurology/stroke setting preferred.
- Demonstrates strong communication and interpersonal skills with the ability to work collaboratively with patients/families, interprofessional team members, and community stakeholders through face to face or telephone interventions, class teaching, group facilitation, adult learning.
- Skillful implementation of planned social work interventions, both individually and in group settings, with the patient, family, caregiver, inter-disciplinary team and community resources.
- Able to address the unique health, social and service challenges for the patient/family/caregiver in keeping with Canadian Stroke Best Practice Guidelines.
- Demonstrates a high degree of initiative, self-direction and ability to multitask in a fast paced environment.
- Demonstrated ability to function effectively as a member of the health care team.
- Satisfactory employment and attendance record.
- Comprehensive knowledge of community resources and research skills to secure new resources.
- Care passionately about the needs and well-being of patients, families and caregivers and the needs of our local community.
