Social Worker

Halton Healthcare is seeking a skilled Social Worker to join our Critical Care Interprofessional team. As a key member of our team, you will provide essential psychosocial support to patients and their families, ensuring a seamless transition back to their community or institutional setting.
Key Responsibilities- Conduct comprehensive social work and psychosocial assessments on inpatients, utilizing various modalities and adhering to professional standards, clinical policies, and practices.
- Develop, document, and implement care plans for hospital discharge, ensuring safe transition and community integration.
- Participate in team conferences, educational rounds, and interdisciplinary discussions to promote effective patient care.
- Intervene in high-risk situations, providing support and advocacy to patients and families.
- Offer education and guidance on healthcare consent and capacity, as well as community resources and referrals.
- Provide selected therapeutic interventions, supportive counseling, and community outreach services.
- Collaborate with healthcare teams to address complex interpersonal concerns and resolve financial, legal, and workplace issues.
- Bachelor of Social Work (BSW) or Master of Social Work (MSW) degree required.
- Current registration and membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Experience providing social work services in an acute care hospital setting.
- Strong clinical skills, including assessment, counseling, and discharge planning.
- Knowledge of community resources and ability to work collaboratively with community partners.
- Familiarity with relevant legislation, including the Mental Health Act and Healthcare Consent Act.
