Job Summary
We are seeking a skilled Registered Nurse to join our Home and Community Care Support Services team as a Rapid Response Specialist. In this role, you will provide high-quality nursing care to patients in their homes, focusing on the transition from acute care to home care for complex/chronic adults and medically fragile children.
Key Responsibilities
- Conduct in-home nursing visits within 24 hours of hospital discharge to assess patients with complex/chronic diseases.
- Complete a comprehensive clinical assessment and confirm the patient's hospital discharge plan.
- Perform medication reconciliation and review medication protocols with patients and caregivers.
- Utilize a 'teach back' approach to educate patients and families on care plans, treatments, and symptom management.
- Initiate communication with primary care providers and ensure follow-up within 7 days.
- Refer patients to longer-term community programs as needed.
Requirements
- Registered Nurse in good standing with the College of Nurses of Ontario.
- Minimum 3-5 years of clinical experience as a Registered Nurse.
- Recent experience providing direct nursing care in Paediatrics, Chronic Disease Management, Medicine, or Cardiology.
- Superior clinical assessment skills and ability to work independently.
- Strong critical thinking and problem-solving skills, effective interpersonal and communication skills.
- Ability to provide health coaching and disease-specific education to patients.
- Advanced oral and written proficiency in English is essential.
- Valid driver's license and access to a vehicle for travel to patient homes.
- Valid Emergency First Aid Certification.
Preferred Qualifications
- Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, and geriatrics.
- Case management experience or recent related community experience.
- Ability to speak French or another second language.
