Registered Nurse (RN) - Community

ParaMed Home Health Care is seeking a compassionate and skilled Registered Nurse (RN) to join our team in providing high-quality care to patients in their homes. As a Registered Nurse (RN) in our community nursing role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:
  • Conduct thorough assessments of patients' needs and develop personalized care plans
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and palliative care
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
  • Document patient progress and maintain accurate records
Requirements:
  • Current registration with the CNO as a Registered Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in delivering community care is an asset
  • Reliability, flexibility, and a passion for making a difference in your community
