Job Title: Patient Care Coordinator

We are seeking a skilled and compassionate Patient Care Coordinator to join our healthcare team at Prince Rupert Regional Hospital. As a key member of our team, you will play a vital role in ensuring the efficient admission, discharge, and transfer of patients, while promoting quality care and patient satisfaction.

Key Responsibilities:
  • Coordinate patient care activities, including admission, discharge, and transfer processes
  • Collaborate with healthcare teams to ensure seamless patient care
  • Develop and maintain effective relationships with patients, families, and healthcare providers
  • Provide exceptional customer service and support to patients and families
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
  • Registration with BC College of Nurses and Midwives as a practicing RN registrant
  • Five years recent acute care experience with one year's supervisory experience or the equivalent combination of training and experience
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Physical and mental ability to perform duties of position
