Mobile Mental Health Worker

We are seeking a highly skilled and compassionate Mobile Mental Health Worker to join our team at Good Shepherd Centres. As a key member of our Primary Care Access and Attachment Team (PCAAT), you will play a vital role in connecting with vulnerable populations and increasing access to primary care.
Key Responsibilities- Provide rapid crisis assessment and development of risk management plans for individuals in need of mental health support.
- Offer crisis intervention and de-escalation services, exercising good judgment and risk management in high-pressure situations.
- Participate in risk assessments and seek consultation as needed to ensure the safety and dignity of all clients.
- Collaborate with PCAAT to develop individualized plans, driven by client self-identified needs and goals.
- Support clients in addressing pressing needs through emotional support, skill training, creative problem-solving, and practical assistance.
- Facilitate and participate in case planning with clients, program staff, and relevant support systems to ensure an integrated approach to service delivery.
- Provide crisis counseling, education, and support to clients and their support systems, tailored to their level of knowledge and understanding.
- Link clients to community resources and provide flexible, time-limited support and advocacy to facilitate community tenure and crisis resolution.
- Maintain up-to-date knowledge of community resources, internal policies, and agency protocols, sharing relevant information with staff.
- A degree or diploma in social work, social service work, nursing, or a related field, plus 2 years of experience in community mental health, addictions, and/or crisis intervention.
- Extensive knowledge of community resources and supports in Hamilton.
- Demonstrated knowledge and skills in suicide risk assessment and behavior management.
- Demonstrated knowledge of crisis intervention, de-escalation strategies, and interventions, with the ability to remain calm in crisis situations.
- CPR/First Aid certification and WHMIS training or willingness to be trained.
- Experience working with mental health, addiction, and concurrent disorders.
- Experience and sensitivity in working with culturally diverse populations, including individuals newly arrived in Canada.
- Commitment to intersectional anti-racist/anti-oppressive practice, harm reduction, and trauma-informed care.
- Ability to work independently and as part of a team.
- Outgoing and adaptable, with a willingness to work in a fast-paced environment.
- Creativity and flexibility in achieving desired outcomes.
- Hospitable and respectful in dealings with clients and stakeholders.
- Strong written and verbal communication skills.
- Good interpersonal and organizational skills.
- Ability to work with various record-keeping technologies, including Electronic Medical Record programs.
