Hamilton Health Sciences is seeking a highly skilled and compassionate Social Worker to join our Pediatric Mental Health team. As a member of our interdisciplinary team, you will provide expert clinical practice, education, consultation, and research to promote excellence in pediatric patient and family-centred care.
Responsibilities:- Provide social work assessment and treatment as part of an interprofessional team
- Develop and implement comprehensive individualized care plans for children and adolescents
- Work collaboratively with other team members to provide evidence-based treatment or best-practice therapies
- Manage and maintain an active caseload as per program requirements and expectations
- Provide case management for patients and families
- Consult and liaison with caregivers and community agencies and community partners
- Master's Degree in Clinical Social Work from an accredited School of Social Work
- Current registration with the Ontario College of Social Workers and Social Service Workers
- Minimum of 5 years clinical experience in a tertiary pediatric mental health setting and experience with gender affirming treatment
- Demonstrated expertise in assessment, psychoeducation, crisis intervention, and advocacy skills in working with children, teens, and their families experiencing health and mental health issues affecting child and family functioning
- Demonstrated expertise in a range of evidence-based, therapeutic modalities (i.e. cognitive behaviour therapy (CBT), motivational interviewing (MI), Emotion Focused Family Therapy (EFFT), Acceptance and Commitment Therapy (ACT), Dialectical Behavior Therapy (DBT), mindfulness)
- In-depth knowledge of trans-specific healthcare needs and social determinants of health impacting trans, non-binary, and gender-diverse communities in Hamilton
- Excellent assessment skills using bio-psycho-social approach
- Demonstrated knowledge with the Mental Health Act, Consent to Treatment Act, Child and Family Services Act, and World Professional Association for Transgender Health guidelines
- Demonstrated excellent interpersonal communication skills, critical thinking, mentoring, and coaching within an interdisciplinary team
