About the Role

The Healthcare Resource Coordination Specialist plays a crucial role in advocating for and coordinating essential resources for patients and their families, ensuring a smooth and timely transition or discharge process. This position involves collaborating with clients whose primary healthcare needs can be addressed by connecting them with appropriate resources such as housing, transportation, financial assistance, and medication support.

Key Responsibilities

  • Work under the guidance of the Program Manager to facilitate resource access for clients.
  • Collaborate with social workers, healthcare teams, and community organizations to provide comprehensive support.
  • Serve as a consultant and educator for clients, families, and healthcare providers regarding available resources.

Qualifications

  • A Bachelor’s degree in Social Work.
  • Registration with the relevant professional body.
  • A minimum of two years of pertinent clinical experience.
  • Strong understanding of organizational policies and procedures.
  • In-depth knowledge of community resources.
  • Ability to establish collaborative relationships with stakeholders.
  • Proficient computer skills, including data management and communication tools.
  • Experience in program evaluation.
  • Excellent communication, organizational, and interpersonal skills.
  • Proficiency in additional languages is an asset, with a preference for French.

Work Schedule

  • This is a full-time, short-term assignment requiring 75 hours bi-weekly.
  • The position is expected to last approximately two months.

Compensation

The hourly wage for this position is $42.89.
