Job Opportunity: Home Health Care Specialist

Bayshore Healthcare is seeking a passionate and caring Home Health Care Specialist to join our inclusive team. As a Home Health Care Specialist, you will be creating meaningful relationships with clients and providing personalized care in the comfort of their own homes.

You will have the opportunity to work with a variety of clients, each with unique needs and circumstances. Your strong communication skills and ability to work independently will be essential in providing high-quality care and support.

Key Responsibilities:

  • Providing personalized care and support to clients in their homes
  • Creating and implementing customized care plans to meet the unique needs of each client
  • Building strong relationships with clients and their families
  • Collaborating with healthcare professionals to ensure comprehensive care
  • Maintaining accurate records and reporting any changes or concerns to the care team

Requirements:

  • Completed a formal, accredited caregiver training program and/or equivalent education and experience
  • Current Standard CPR and First Aid Certificate
  • Clear Criminal Record Check with Vulnerable Sector Screening within the last 6 months
  • Ability to travel from home to home within the community
  • Strong verbal and written English communication skills required
